What is SystmOne?
SystmOne is a clinical record system used to document patients’ care electronically (known as your health care record). SystmOne is used across the country by many GP practices and other health and care services – for example district nurses, community therapy teams and ambulance services.
How does it work?
Whenever you are in contact with the Specialist Palliative Care Service in the community, hospice, or hospital (whether it be a visit, out-patient appointment, telephone call or email) all relevant clinical information will be documented in your record on SystmOne. This information can be shared with other health and care professionals who are caring for you to ensure they are fully informed about your medical history should you choose to allow sharing.
What are the benefits?
Using SystmOne will ensure that the health and care professionals caring for you will have the most up to date information to provide you with the most appropriate care and improve understanding between services.
Sharing your clinical information can remove the need for you to repeat your medical information multiple times to different health services.
It will help to avoid unnecessary delays in the Palliative Care team liaising with GP’s and other health care services using a paperless system, for example Palliative Care requesting medication from GP’s or requesting District Nurse urgent visits.
Who can access my record?
Only health and care services who are treating you and use SystmOne will be authorised to access your SystmOne record. This also includes administrative staff within these services who will have limited access to your health records, to enable them to perform relevant administrative tasks.
If you access another health or care service, they will require your consent to see any previous medical history or notes (unless legally required).
Do I have to share my record?
It is your choice if you do not wish for all, or part of your clinical record to be shared.
What happens if I do not want to share all, or part of my clinical record?
If you do not wish to share ANY of your Salisbury Hospice clinical record with other health organisations, you can request that your information is restricted. In which case your Salisbury Hospice clinical record will only be shared within Salisbury Hospice Palliative Care Services.
However, you must be aware that without knowledge or details of palliative input by other services, your palliative care could potentially be delayed or impacted upon.
If your clinical record contains private or sensitive information that you have shared with the Palliative Care Team but do NOT wish for this information to be shared with other health or care services you can request for ONLY this information to be kept private, whilst allowing the rest of your clinical record to be shared.
What happens now?
If you DO NOT wish to share all OR part of your palliative care clinical record please inform us of this decision as soon as possible either verbally or in writing. We can then take the appropriate action to ensure your palliative care record is NOT shared outside of the Specialist Palliative Care Service.
Who do I contact to discuss, consent, or withdraw consent for my record being shared?
You can do these 2 ways:
1) Discuss with your allocated Community Clinical Nurse Specialist if you have one – they may have already discussed this with you.
2) Contact Salisbury Hospice directly via the contact details below and inform the administration team who will inform the relevant health care professional within the Hospice.
